What the form asks for

1. Your full names and national registration number. 2. The address where you live. 3. Your statement: what you are swearing or declaring to be true, written out in full on the numbered lines. 4. Your signature, under the line "I make the above statement conscientiously believing the same to be true". 5. The place and date, and the signature and stamp of the Commissioner of Oaths.

Before you sign

- Do not sign in advance. The form is signed in front of the Commissioner of Oaths, who then signs, dates and stamps it. - Take your national ID so your name and number can be checked against it. - Write only what you know to be true. An affidavit is a sworn statement, and a false one is an offence. - Strike out whichever of "swear" or "declare" does not apply. - Check whether the office asking for the affidavit wants its own wording. Some institutions only accept their own form.
